Physical therapy (PT) can be challenging, especially if you haven't been very active due to surgery. However, a good physical therapist will encourage you to challenge yourself while respecting your comfort levels. They will assess your condition, test your strength or range of motion, and help you prioritize which problems to work on. Even gentle exercises and light physical therapy can help reduce pain and improve physical function.

In my experience, no.
Post surgery, typically you are supposed to be inactive while things heal for a bit, then straight to PT.
The purpose of PT is to begin light muscle, nerve, and tendon growth.
As long as the PT and Doctor are aware of your situation, they can coordinate the best steps for you.
ACL surgery required the physical therapist to be aggressive on the muscles within reason.
However different surgeries may require different PT workers.

I have had PT for both knee replacements, my neck, my rebuilt foot and my shoulder. I can’t say that it hurt or that it was even uncomfortable. I myself have had no pain at all. Good luck. Let me know how it goes❤️
